In February the government will be sending a letter to every household regarding care.data (see here for more info). If they receive notification that the person no longer lives at that address then a process to remove patients from our list will start. This is outside of our control.
Please make sure we have your correct contact details. You can do this via reception, or by using the Patient Access link on the top right hand side of our website (you can ask reception for a PIN if you don’t have one already.)
