In order to register at the practice you need to live within our catchment area, as outlined on the practice area map (left). The practice boundary crosses certain postcodes, so if you are unsure from looking at the map you can telephone or visit us and the reception staff will be happy to check for you.
If you wish to register with the Practice you should present at any of our 4 sites, Brownlow, Ropewalks, Student Health or SHAC (University of Liverpool Students only) where you will be given 2 forms to complete: a GMS1 (registration form) and a New Patient Information Form. Or you could print these forms off from this website and bring them with you.
Once the above forms have been completed the reception staff will enter your details onto our medical database and make an appointment for a new patient check if required
All new patients will be given a copy of our Practice Leaflet and will be offered the opportunity to be issued with a PIN number that will allow you to register with the Online Services website. This will enable you to make and cancel appointments, update your contact details, and order repeat prescriptions on-line.
We endeavour to process registrations immediately to enable a smooth transition from one practice to another and to allow patients to access our services.
