This is a new position and exciting opportunity for you to develop this role. The post holder will be a highly experienced Practice Nurse/ Long Term Conditions Nurse.

This is a dual clinical and non- patient facing role. In addition to your clinical role you will also lead and manage our team of LTC nurses and will work closely with the Quality Lead, Site managers, recall team and IT team to ensure all our sites are working consistently towards QOF targets and are reached by the desired timescale. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. As an autonomous practitioner the nurse will be a non-medical prescriber able to demonstrate critical thinking and decision-making relating to Long term conditions.
