Job summary
Due to expansion within Brownlow Healths Homeless Service, we have a fantastic opportunity for an experienced and compassionate clinician who has experience working with vulnerable adults. The post holder will work with the Hospital In-reach service, identifying homeless patients, ensuring they receive appropriate health and social support to facilitate a safe discharge whilst also providing timely review post discharge to reduce the risk of readmission. We would welcome applications from both Nurses and Occupational Therapists.
