Job summary
Act as first point of contact for non registered carers and patients who frequently attend practice to establish needs in a non-clinical sense and support in obtaining the correct referrals/ level of support for entitlements and social care. To alleviate pressures on GP capacity, streamlining the patients journey to ensure ongoing care is assessed correctly and planned effectively.
